If you searched for a Physical Therapist (PT) or Physical Therapist Assistant (PTA) license and did not find a licensee, check the PT Compact Commission Verification page to see if the PT or PTA holds a compact privilege in Delaware.

If you are requesting a license verification for someone other than yourself, use the License Verification Request Form to request an official license verification. Follow the instructions on the form to complete your request. DO NOT use the form to request a license verification of your own license.
To verify a Delaware Physician M.D., Physician D.O., Physician-Limited, Physician Assistant, ACGME Training, or Administrative Medical license, you must submit the request directly through VeriDoc. Attention Licensees: The 2021 license renewal period has ended. You are no longer able to renew your license. If you wish to maintain licensure in the State of Nevada, you will be required to reinstate your license. You must submit proof of the required CME/CE with your application for reinstatement.
This means that you no longer may practice medicine, respiratory care, and perfusion until you have a current license or certificate in the State of Nevada, and the unlicensed practice of medicine, respiratory care, or perfusion is criminal conduct under Nevada law.

To verify a supervised temporary license, this Directory serves as a primary source verification of the credential issued by the State of Arizona Board of Psychologist Examiners. Emergency temporary licenses for Psychologist and Behavior Analyst independent practice were issued between 04.09.2020 through 09.30.2022 in response to the state of emergency due to COVID-19. The Board ceased accepting applications for this temporary license as of 10.01.2022. Under A.R.S. 32-3124(D), a temporary license is no longer valid upon issuance or denial of a full, independent Arizona license. All active emergency temporary licenses will expire on 01.01.2023 per Senate Bill 1309 signed by Governor Doug Ducey on 03.25.2022. Pursuant to A.R.S. 32-3226, licensees who do not have a business address may OPT OUT of the requirement to make their home address public (A.R.S. 32-3801). Licensees who opt out must provide a phone number or email address to the Board which can be disclosed to a patient or client who is seeking a copy of their medical records. If you fall into this category and wish to opt out, please email the Board office with a written request to opt out per A.R.S. 32-3226.

The West Virginia Board of Medicine is the sole authority for the issuance of licenses to practice medicine and surgery, to practice podiatry, and to practice as a physician assistant in collaboration with MDs and DPMs in the State of West Virginia, and is the regulatory and disciplinary body for medical doctors, podiatric physicians and their physician assistants.
Board Members Board members are appointed by the Governor to three-year terms. The board consists of nine members, three of whom shall be lay members, one of whom shall be a counselor educator, and five of whom shall be professionals actively engaged in professional counseling or marriage and family therapy and broadly representing a cross section of the licensed disciplines governed by this board. No member may serve more than three consecutive terms.

Licensees are not required to carry or display a renewal certificate or wallet card. An optional, informal wallet card may be printed from the Last Name/DOB search results page by licensees who wish to carry a card. If a card does not print properly due to browser, platform or printer issues, the agency is not able to provide technical support.
